This happened to me as a result of activating the High Stakes tournament. As long as that quest has been initiated (hopefully that's not the case), those doors will stay locked and the Madame will not appear where she normally would (by the steps). Unfortunately, you won't be able to initiate a deadly plot, as long as high stakes is active.

After that you have to go somewhere secluded, Usually where there is nekkers watching. Nope, only in the first game. Witcher 1 was best for this, you could have sex with almost any female in-game.

Does Yen/Triss/Ciri show up in any of the expansion packs?
Yen shows up on the last quest in Blood and Wine if you romanced her. Triss when you romance her. Ciri shows up if you didn't romance either of 'em or 'effed it up.
